Recalcitrant hypocalcemia in a lactating female after total thyroidectomy for papillary thyroid carcinoma
We present a case report of a lactating woman with papillary thyroid carcinoma requiring total thyroidectomy and neck dissections.Postoperatively the patient suffered from recalcitrant hypocalcemia which necessitated multiple medical maneuvers to rectify. The medical treatment required to correct the calcium levels is described herein.With the incidence of thyroid cancer on the rise and the increased identification of thyroid lesions during pregnancy, the number of lactating women undergoing thyroidectomy will likely increase. Maternal hepatitis B virus or hepatitis C virus carrier status as an independent risk factor for adverse perinatal outcome
To examine the impact of maternal hepatitis B virus (HBV) or hepatitis C virus (HCV) carrier status on pregnancy outcomes. A population-based study was performed by comparing all pregnancies of HBsAg and/or anti-HCV seropositive women who delivered during the years 1988[ndash]2007 with all other pregnant women who delivered in the same period. Multivariable logistic regression models were constructed to control for confounders. Seven hundred and forty-nine hepatitis seropositive pregnant women were identified out of 186 619 deliveries (0.4%). Exposure to methyl isocyanate and other toxic gases in Bhopal, India, on December 3, 1984 resulted in thousands of acute deaths, pregnancy loss and long-term effects.From 1985 to 2007, we conducted successive surveys of vital status and health to determine whether the exposure of parents to toxic gases in the Bhopal incident affected the 5-year survival and anthropometric variables of their offspring.Initial 5-year mortality of offspring of exposed parents was very high. Male but not female offspring who were exposed to gases in utero or who were born to exposed parents were stunted in growth until puberty, which was followed by a period of accelerated growth. Gut microbiota composition is associated with body weight, weight gain and biochemical parameters in pregnant women.
The objective of the present study was to establish possible relationships between gut microbiota, body weight, weight gain and biochemical parameters in pregnant women. Fifty pregnant women were classified according to their BMI in normal-weight (n 34) and overweight (n 16) groups. Gut microbiota composition was analysed by quantitative real-time PCR in faeces and biochemical parameters in plasma at 24 weeks of pregnancy. Reduced numbers of Bifidobacterium and Bacteroides and increased numbers of Staphylococcus, Enterobacteriaceae and Escherichia coli were detected in overweight compared with normal-weight pregnant women. Latent tuberculosis among pregnant mothers in a resource poor setting in Northern Tanzania: a cross-sectional study
Conclusion:
The prevalence of latent tuberculosis in pregnant women was found to be relatively high compared to that of the general population. In endemic areas, socio-demographic parameters alone are rarely adequate in identifying women susceptible to TB infection; therefore targeted screening should be conducted for all pregnant women at high risk for activation (especially HIV positive women). As opposed to the current policy of passive case detection, there appears to be an imminent need to move towards active screening. Ethnicity may provide important clues into genetic and cultural differences which predispose to latent tuberculosis, and is worth exploring further. (Source: BMC Infectious Diseases)

Interim analysis of a randomized clinical trial comparing abdominal versus transvaginal ultrasound-guided embryo transfer
Conclusions: Neither transabdominal nor transvaginal ultrasound-guided embryo transfer is more beneficial in optimizing pregnancy outcomes. While delays often occur while waiting for bladder distension for transabdominal ultrasound-guided embryo transfer, uterine position, parity and level of physician comfort should dictate the choice of either approach.
